Get your free care roadmap in six questions
The assessment asks the six questions a nurse asks first. No card is required. Nothing is sold. You can retake it whenever the situation changes.
What are the six questions?
- Where is care needed, so costs can be matched to the local market
- Who is care for: a parent, a spouse or partner, or yourself
- How old is the person who needs care
- What help do they need day to day
- What diagnosis are you dealing with
- What just changed, such as a hospital discharge or a fall
What do you get at the end?
- The level of care the situation suggests, in plain English
- Typical local cost ranges for that level of care
- The benefit programs the answers point to, including Medicare, Medicaid, VA and EEOICPA
- A numbered list of steps to take this week
How long does the assessment take?
Most families finish in about ten minutes. You can stop and come back. Nothing is shared with a care facility or an agency, and CareBridge Navigator takes no referral fees for placement.
